For Eligible Neighborhood INTEGRITY (Medicare-Medicaid Plan) Members
You will have a care manager to help coordinate your care. The Neighborhood INTEGRITY care managers will help you coordinate your care team. Your care manager will ensure your medical, behavioral health, substance use, long-term services and supports (LTSS), social and transportation needs are coordinated. Your care manager will coordinate these services and include them in your care plan.
Your care manager will also:
- Provide you with education about conditions, medications, treatment and resources in your community.
- Provide you with education on available benefits and services, help address any medication issues, create self-management plans and goal setting.
- Help you coordinate and facilitate your medical services and provide assistance in adhering to your plan of care with your provider.
- Provide assistance to help you find specialists and get any durable medical equipment (DME) you may qualify for.
- Enhance communication between multiple providers that may be involved in your care.
What is a care manager?
A care manager is a qualified individual who helps you manage all of your providers and services. He or she works with you and your care team to make sure you get the care you need and stay as healthy as possible.

How can you change your care manager?
