For our valued Medicaid members:
On April 15, 2022, Neighborhood became aware of a breach of Medicaid member protected health information (PHI). This means that the names and ID numbers of some of our members may have been shared with another person by mistake.
What happened?
- Some Neighborhood member ID cards were mailed to the wrong address.
- You may have received a card that was not yours.
- Your ID card may have been sent to someone else.
What information was shared?
- Your name and Neighborhood member ID number may have been shared.
How is Neighborhood fixing the problem?
- Neighborhood has taken steps to protect your information by deactivating all ID numbers that were mailed to an incorrect address. We have sent new member ID cards with this number to members who need them.
- Neighborhood is making sure that our process is fixed so this problem does not happen again.
What members need to do:
- Please cut up and throw away any ID card you received that does not have your information on it.
- If your card was sent to the wrong person, you should have already received a new ID card. If you have not, please contact us.
- When you receive your new ID card, contact your health care providers and your pharmacy to update your member ID number.
Neighborhood cares about our members and apologizes for any inconvenience this may have caused.
We are offering optional identity protection assistance to members who were affected by this breach at no cost for twelve months. More information about this assistance will be sent directly to members.
